**Job Summary and Responsibilities** Pharmacist - St. Bernardine Medical Center Join a mission-driven healthcare team that prioritizes patient safety, professional collaboration, and compassionate service. St. Bernardine Medical Center is seeking a skilled and detail-oriented Pharmacist to provide comprehensive pharmaceutical care across inpatient services. In this vital role, you will ensure safe medication practices, participate in performance improvement initiatives, and serve as a trusted resource for clinical staff and providers. Key Responsibilities: + Review, process, and dispense medications and IV therapies with precision and in compliance with all hospital policies and regulatory guidelines + Provide direct clinical consultation and medication therapy recommendations to physicians and nursing teams + Supervise and support pharmacy technicians in compounding, repackaging, and medication distribution + Monitor patient medication profiles for drug interactions, allergies, and contraindications + Perform pharmacokinetic calculations and adjust dosages as appropriate for patient age, weight, and condition + Support formulary compliance, therapeutic interchange, and clinical pharmacy initiatives + Participate in department performance improvement projects, medication use evaluations, and adverse drug reaction monitoring + Ensure accurate and timely documentation within the EMR and pharmacy systems + Maintain compliance with controlled substance protocols, regulatory standards, and documentation requirements This position requires the full understanding and active participation in fulfilling the Mission of St. Bernardine Medical Center.
