Job Description

We are currently looking for a Treatment Coordinator to direct and monitor the daily operation of assigned unit in order to assure active programming, and provide ongoing staff training, and supervision of assigned staff. Responsible for the development, implementation and coordination of services and supports and ongoing monitoring of the needs of the program and services.

Essential Duties And Responsibilities

  • To ensure all individuals serviced are treated with dignity and respect, thereby, facilitating agency philosophy and mission, adhering to principles of family-centered care.
  • To adhere to and ensure assigned staff adhere to all agency policies.
  • Primary liaison with all families and their providers.
  • Coordinates intake as prescribed by DHS standards.
  • Gathers child specific information in order to facilitate HBTS/ABA/PASS/Respite.
  • Ensures that staff are fulfilling work schedules and are in compliance with all agency guidelines as described in the AccessPoint RI employee handbook (e.g. time sheets, incident reports etc).
  • Responsible for the monitoring of staff performance.
  • Supervision of assigned staff, to include regularly scheduled individual supervision meetings, staff meetings and direct support staff performance appraisals.
  • Ensures the co-ordination care by directly communicating with all relevant caregivers and others involved in the child’s plan of care.
  • Treatment Coordinators are responsible to maintain the following contemporaneous records:
  • Contact sheets for all meetings and interactions with families, child, staff, and collaborating agencies. These records should include the following information.
  • The reason for the contact and any relevant history related to the contact.

(ii) Type and location of contact.

(iii) Date, time, and duration of the contact.

(iv) Relevant current status.

  • The nature of supports provided and their relation to current goals and objectives.

(vi) Outcomes of contact and/or recommendations.

(vii) Legible identity/credentials of care provider.

  • Maintain meeting minutes for all staff and team meetings.
  • Responsible for the creation of a weekly data summary from the individual data sheets for each child and their respective objectives for review by the treatment consultant and clinical supervisor.
  • Maintaining all case records sent by other providers that also provide support our clients (ex. IEP’s, OT, PT, Speech, Therapists, etc.)
  • Responsible to forward all collected relevant information to the Clinical Supervisor, Treatment Consultant, and Program Director as required.
  • Schedules meetings and supervisory sessions for the Clinical Supervisor and Therapeutic Consultant.
  • To initiate, document and address disciplinary matters in conjunction with the Director of Human Resources and the Program Director as applicable.
  • To provide and facilitate staff training on an ongoing basis and ensure completion of all required staff training with all assigned staff.
  • Facilitate team meetings and reviews, as applicable.
  • Review and assist direct support staff in planning and implementing schedules, as applicable.
  • To provide and participate in providing and facilitating staff training on an ongoing basis and ensure completion of all required staff training with all assigned staff.
  • To complete and review all documentation including daily and monthly summaries, expense ledger, petty cash slips, data sheets, mileage, etc.
  • Develop community relationships and contacts that promote and facilitate meaningful inclusion of individuals in community-based activities.
  • Assist in the hiring process, making recommendations to Program Director.
  • Work in conjunction with the clinical department to ensure the provision of appropriate health/medical clinical supports.
  • Follow, direct and implement all health/medical and clinical service programs as prescribed and in-serviced.
  • Ensure all consumers are informed of their rights in a manner consistent with their individual style of learning. Ensure protection of consumer’s rights. Inform Office of Quality and Standards of pertinent issues as mandated.
  • Other duties as assigned.

Minimum Qualifications

  • BA/BS or eligible under variance provisions of current DHS standards. At least 3 years experience supporting with individuals with special needs. At least one-year supervisory experience preferred. Valid driver’s license with acceptable driving record.
  • Willingness and ability to complete CPR, NCI, Human Rights and Management Training. Access to and provision of an insured automobile on an ongoing basis.
  • Has successfully passed BCI and CANTs.

Necessary Knowledge, Skills & Abilities

  • Ability to communicate effectively both in written and oral formats.
  • Ability to read, comprehend policies and instructions, and use varying degrees of independent judgment and discretion.

Essential Physical Demands

  • Lifting and carrying one hundred (100) pounds with two people.
  • Lifting and carrying fifty (50) pounds.
  • Standing/bending 80% of shift.
  • Pushing/Pulling 70% of shift.
  • Ability to carry out NCI techniques and holds
